This form is to be used for reporting Non-Urgent Maintenance Requests at Telstra Network Sites:

If your issue relates to a Network Critical issue, please call the following emergency number: 1300 652 235 and select appropriate option relative to your issue.

Network Critical is defined as follows: The problem is:

  • currently impacting customers and network equipment or will impact customers and network equipment in the near future;
  • a high risk to health and safety (if so, relevant HS&E controls and Donesafe is required to be implemented immediately by reporter)
